Build a concise health-information pack

Bring the information the provider has asked to see. This may include recent reports, imaging, referral letters, a medication list and the contact details of other treating professionals.

Arrange documents by date and place the most recent summary first. Avoid sending sensitive records through an unapproved channel; ask the receiving facility how it accepts files and whether original imaging is needed.

Write questions in priority order

Appointments can feel fast, especially when the subject is stressful. Put the most important questions first and use plain language. Ask who will explain results, how follow-up will be arranged and which warning signs require urgent help.

If someone is attending with you, agree on their role. They might take notes, listen for next steps or help describe the timeline. The patient should remain central to the conversation and consent to information sharing.
