Put consent before convenience
Ask the patient who may receive information and make practical arrangements. Healthcare providers may require written consent, identification or another formal process.
A relative’s good intention does not replace the patient’s privacy rights. Clarify permissions before requesting records or detailed updates.
Choose one coordination lead
Nominate one person to speak with the local coordinator and provider contacts. That person can update the wider family after information has been confirmed.
Use a shared action list for appointments, documents, payments, transport and follow-up. Mark each item with an owner and date.
Separate facts from family discussion
Record what the provider has confirmed separately from questions, opinions and family decisions. This helps prevent an assumption from being repeated as a clinical instruction.
Medical concerns should return to the treating team. A non-clinical coordinator can organise the question and contact route but should not interpret the answer.
Prepare for transitions
Hospital admission, discharge and a return home are points where information can be lost. Ask for the practical plan, provider contact and required follow-up before each transition.
If the patient will travel after care, confirm timing and fitness to travel with the clinical team rather than relying on a family timetable.
